• Fully accomplished under oath Personal Data Sheet (CSC Form No.212, revised 2017) with recent passport sized picture taken within the last six (6) months, and the duly accomplished Work Experience Sheet (WES), both of which can be downloaded at www.csc.gov.ph. The PDS and WES should be converted into a single PDF file.
• Performance rating in the present position for one (1) year, if applicable
• Certificate of eligibility/rating/license
• Transcript of Records and Diploma
• Certificates of Learning and Development interventions/Training programs converted into a single PDF file
